I see few possibilities:
mechanical issue (stuck gas, brake won't engage), medical issue (diabetes are known to cause this), and just flat out hostile driver who can't drive nicely.

Probably medical issues. Doesn't look like intentional ramming. I had a coworker die while driving in the middle of the day. Suffered some sort of heart problem, he was in his 40's and was super fit. He was unconscious, went down an off ramp, crossed traffic and slammed in a tree. He was probably already dead before the tree impact.

Automotive pinball.
Must happen fast, when my friend "died" behind the wheel of his Dodge ram charger he was waiting for green light, and he cant tell if he felt it coming or he just keeled over at once and was gone.
But then again most of that day is gone to him, he died several times at the site and a few times on route to the hospital, but they managed to kickstart him every time.

Thats the bad thing with auto transmission, if you keel over you start going, i don't think a stick shift is prone to do the same.
